REGISTRATION INFORMATION

Registration is open to all EGA members.

All seminar participants must be registered at the Doubletree At Warren Place in Tulsa or pay a one-time $50 commuter fee. Participants should contact the hotel directly to make individual reservations.

Registration opens on June 8 at Bella Vista 2004 Seminar.
"First Day Registration" June 8 – 11, 2004
"Second Day Registration" November 1, 2004 - January 31, 2005
February 1 - May 15, 2005 will be "first-come, first-served" basis.

Payment:  Full payment is due by January 31, 2005 for "first-day" and "second-day" registrants.  Registrations postmarked after January 31, 2005 must include full payment.  "First-day" registrants may pay in two installments (1/2 by November 1, 2004, and 1/2 by January 31, 2005).  Payment should be made by check or money order to “SCR Seminar 2005."  No credit card payment is available.  Please enclose a self-addressed stamped envelope with your registration.  A separate invoice will be sent to you with your class assignment and total fees due.

Prior to May 1, 2005, your registration cost will be refunded minus the non-refundable registration fee of $40.00.
After May 1, 2005, there will be no refunds.

All fees include three days of classes, three lunches, two dinners plus one banquet dinner, entertainment, favors, project bag, door prizes and
LOTS OF FUN!

Registration forms may be reproduced. Be sure to keep a copy for your records.
