Please complete (PRINT) the entire registration
form and be sure to keep a copy for yourself!!
Please include a business sized, self addressed, stamped envelope
at the time of your registration or you may pay $1.00 and we will provide
the SASE.
The Basic
Registration Fee includes classroom/studio space, food, favors,
brochure and mailing, and administration costs. The additional fees
for the classes are for teacher expenses.
Things are a little different this year, so please read these instructions!!
Class
Selection
You may pay the
Basic Registration Fee and enjoy all Seminar Activities including three
days of Studio Time to work on UFOs. You also have the option of taking
three days of any combination of classes and/or Studio Time. There are
additional fees for the classes to cover teacher expenses. Because of
the nature of our classes, we need to be able to balance the class sizes
to maximize your enjoyment of the teachers' knowledge. Therefore, we
cannot accept requests for placement in certain classes at certain times.
First Choice: Please list up to three days of classes. If there
are not three days of classes listed, you will automatically be assigned
to Studio Time for the remainder of the three days at no extra charge.
Everyone will be registered for three days.
Alternate Choices:
We may be unable to place you in one or more of your First Choices
because the class is full or does not make. Please list your alternate
choices in the order you would like them to be considered.
Notes:
Please explain any preferences about your alternate choices. You may
attach an additional piece of paper if you wish.
Hotel
All
participants must be registered at the Hilton Arlington or pay a one-
time $100.00 facilities use fee. Please see the Hotel Information page
in this brochure for further details.
Payment
- A non-refundable
registration fee of $50.00 is due at time of registration. All First
Day Registrants will receive a 20% discount on this fee. All who register
in San Antonio beginning June 11 and all forms postmarked by June
27, 2008 will be considered First Day Registrants.
- You will receive
an invoice in September with your class assignments and your total
balance due.
- Registrants may
pay in two installments due November 15, 2008 and January 31, 2009.
- Full payment
is due by January 31, 2009.
- Prior to May
1, 2009 you may receive a refund of your Basic Registration Fee and
any Class Fees.
- Checks should
be made out to SCR Seminar 2009.
- Registration
forms may be reproduced; in fact please make a copy of your form for
your records before turning it in.
